Michael Jay Surbaugh, age 56, passed away April 19, 2016 in Tulsa, Oklahoma. Michael was born September 29, 1959 to John Lewis and Sarah Esther Surbaugh in Springfield, Ohio. He worked for the Oklahoma Department of Human Services as a programs assistant administrator. Survivors include his brother John Surbaugh along with other family and friends.
